We are currently seeking a compassionate and motivated Nurse Practitioner (NP) to join our growing team serving Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALFs) throughout Fairfield County, CT. Your Role:
- Provide psychiatric evaluations, medication management, and consultative services to geriatric patients in long-term care settings
- Collaborate closely with facility staff and families to develop and manage care plans
- Deliver care that improves quality of life and mental wellness for aging populations
Ideal Candidate:
- Board-Certified Nurse Practitioner (PMHNP preferred, but not required)
- Experience or strong interest in geriatric mental health
- Excellent communication and teamwork skills
- Ability to work independently across multiple locations in Fairfield County
