Qualifications:
- Bachelor’s degree in Social Work or a human services-related field (such as Psychology, Sociology, Special Education, or Rehabilitation Counseling).
- Minimum of one year of supervised social work experience in a healthcare setting working with elderly populations.
- Experience in a Skilled Nursing Facility (SNF) preferred.
- Complete psychosocial assessments and develop individualized care plans.
- Maintain accurate and timely documentation in compliance with state and federal guidelines.
- Provide emotional support and crisis intervention for residents and families.
- Coordinate discharge planning and referrals to community services.
- Advocate for residents’ rights and collaborate with the interdisciplinary care team.
- Educate staff and support compliance and survey readiness.
