CARE Specialist
General Purpose: To manage and stabilize immediate behavioral healthcare needs of assigned caseload prior to offering transition to appropriate, ongoing program prior to closure.
Essential Functions/Duties:
1. Manage and stabilize immediate behavioral healthcare needs of assigned caseload prior to offering transition to appropriate, ongoing program, i.e., Healthy Start, Healthy Families, Nurse Home Visiting, etc., prior to closure.
2. Contact pregnant women, postpartum women, and families of children under the age of three to assess risk factors that may lead to poor pregnancy outcomes and/or poor developmental outcomes.
3. Verify information on Florida Prenatal Screen, Florida Infant Screen, or HSCFV referral.
4. Complete initial intakes to assess service needs.
5. Provide referrals to services and community resources based on identified service needs and risks.
6. Provide information on breastfeeding, parenting education, preventative health education, child development, health insurance, and healthcare options to include prenatal care, pediatric care, primary care, and family planning.
7. Complete screenings and assessments, including but not limited to the following: general health, child development, depression, and substance use.
8. Complete, update, and implement Plans of Safe Care.
9. Participate in multi-disciplinary staffings with community partners to coordinate care for assigned families.
10. Complete documentation and related uploads in Well Family System (WFS) as initial intakes are completed.
Additional Functions/Duties
Effectively manage administrative duties associated with essential functions.
2. Work outside of normal business hours as needed, i.e., early morning, evening, and weekend.
3. Attend meetings, events, trainings, and conferences to include related travel.
4. Perform other duties as designated by assigned supervisor.

5. Knowledge and adherence to pregnancy and postpartum non-directive information and counseling for prenatal care and delivery, infant care, adoptions and pregnancy terminations options.
6. Knowledge of:
· Community resources for at-risk pregnant and interconception women, infants, young children, and their families and procedures for accessing those resources;
· Available maternal-child health services;
· All components of the Florida Prenatal Screen and Florida Infant Screen;
· Evaluation of service needs and assessments;
· Maternal and child health principles as outlined in HSSG Chapter 4;
· Child abuse and neglect indicators and reporting responsibility to the Florida Abuse Hotline, 1.800.962.2873;
· Home environment and safety assessment;
· Funding resources, i.e., local funding options, eligibility and limits of Medicaid, and local workforce development resources;
· CI&R coding procedures; and
· Situations in which it is necessary to contact a supervisor immediately regarding a family’s immediate needs and/or safety concerns.
Education
Four (4) year college degree preferred in one of the following areas: social sciences; a health-related field such as nursing, health education, health planning, or health care administration. Other education requirements may be considered in lieu of a four (4) year college degree as follows:
· Associate degree and licensure as a RN with three (3) years of public health/maternal child health experience or licensure as a LPN with four (4) years of public health/maternal child health experience.
· Two (2) years of college with three (3) years of public health/maternal child health experience.
· High school education or equivalent with additional support provided by HSCFV as outlined in HSSG Chapter 4.

1. Valid Florida driver’s license with a safe driving record and current private auto insurance.
2. Pre-employment Screening Clearance: Level 2 Background Check, Drug Screening, Local Criminal History Check, and Motor Vehicles Records Check.
