Infection Prevention and Surveillance
- Conduct ongoing surveillance for healthcare-associated infections and communicable diseases.
- Identify, investigate, monitor, and report infection trends throughout the facility.
- Maintain an effective infection tracking and reporting system.
- Ensure timely reporting of reportable diseases and outbreaks to appropriate regulatory agencies.
- Monitor antibiotic usage and assist with antibiotic stewardship initiatives.
- Provide infection prevention education and training to staff during orientation and annually thereafter.
- Conduct focused training sessions as needed regarding infection prevention practices, outbreak management, hand hygiene, and personal protective equipment (PPE).
- Serve as a resource to staff regarding infection prevention and control concerns.
- Ensure compliance with CMS, CDC, OSHA, state health department, and facility infection prevention requirements.
- Maintain current infection prevention policies and procedures.
- Participate in facility surveys and assist with responses to infection control deficiencies.
- Conduct routine audits of infection prevention practices and implement corrective actions when necessary.
- Lead the facility's response to infectious disease outbreaks.
- Coordinate resident and employee testing as required.
- Implement isolation precautions and monitor compliance.
- Collaborate with local and state health departments during outbreaks.
- Participate in Quality Assurance and Performance Improvement (QAPI) activities.
- Analyze infection data and prepare reports for the Quality Assurance Committee and leadership team.
- Develop and implement action plans to reduce infection rates and improve outcomes.
- Monitor employee immunization and screening requirements as applicable.
- Assist with employee exposure investigations and follow-up.
- Provide guidance regarding communicable disease exposures and return-to-work recommendations.
- Current RN or LPN license in the state of employment required.
- Minimum of two (2) years of clinical nursing experience preferred.
- Long-term care experience preferred.
- Infection Preventionist training completed or willingness to complete required training within designated timeframes.
- Certification in Infection Control (CIC) preferred but not required.
- Knowledge of CDC, CMS, OSHA, and state infection prevention regulations preferred
- Strong organizational, communication, and analytical skills.
- Ability to move throughout the facility.
- Ability to observe resident care practices and conduct audits.
- Ability to occasionally lift up to 25 pounds.
