Responsibilities
- Deliver comprehensive personal care assistance, including bathing, grooming, dressing, and mobility support
- Assist clients with Activities of Daily Living (ADLs) such as eating, toileting, and transferring using proper patient transfer methods and equipment like Hoyer lifts
- Monitor and record vital signs accurately while maintaining detailed medical documentation within Electronic Health Record (EHR) systems
- Observe patients closely for changes in condition, behavior, or symptoms related to dementia, Alzheimer's disease, or other chronic conditions
- Communicate effectively with clients, families, and healthcare teams while maintaining strict patient confidentiality per HIPAA guidelines
Qualifications
- Valid certification as a Home Health Aide with current state licensing or registration
- Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring hospice or end-of-life care
- Familiarity with EMR/EHR systems for medical documentation and client records management
- Knowledge of medical terminology, medication administration protocols, vital signs measurement, and basic life support (BLS) including CPR
- Experience assisting with ADLs in assisted living or nursing home settings is highly preferred
- Ability to lift heavy objects safely and perform physical tasks such as patient transfers using Hoyer lifts or other equipment
- Strong communication skills for effective client interaction and team collaboration
