Job Description

As an LPN in our assisted living and memory care community, you will be responsible for providing direct care to our residents, ensuring their comfort and well-being. Your duties will include:

1. Assessment and Diagnosis: Conduct initial and ongoing assessments to determine residents' needs, including physical and mental health, medication administration, and nutritional status.

2. Medication Management: Monitor and administer prescribed medications, ensuring accurate documentation and adherence to medication schedules.

3. Health and Hygiene: Assist with personal hygiene tasks, such as bathing, dressing, grooming, and oral hygiene, to promote and maintain residents' health and well-being.

4. Emotional Support: Provide emotional support to residents, their families, and caregivers, offering reassurance, companionship, and assistance in coping with challenging situations.

5. Nutrition: Monitor residents' dietary needs, assist in meal planning, and assist with food preparation to ensure proper nutrition.

6. Coordination: Collaborate with physicians, therapists, and other healthcare professionals to ensure continuity of care and optimal resident outcomes.

7. Documentation: Complete accurate and timely documentation, including medical records, progress notes, and required reports.

8. Shift Coverage: Work various shifts, including evenings, weekends, and holidays, as needed to provide comprehensive care to our residents.

Qualifications

• Licensed Practical Nurse (LPN): Must hold a valid LPN license in the state of employment.
• Experience: Previous experience in assisted living or memory care preferred.
• Compassionate: Possess a genuine concern for others, with a strong ability to empathize and connect with residents.
• Effective Communication: Excellent verbal and written communication skills, with the ability to interact with residents, families, and colleagues at all levels.
• Problemsolving: Ability to think critically, problemsolve, and make sound decisions in a fastpaced environment.
• Flexibility: Ability to adapt to changing circumstances and work effectively in a dynamic team setting.
• Physical Stamina: Ability to lift and transfer residents, stand for extended periods, and perform other physically demanding tasks as required.
