RAPPORT (Rural Approach to Pregnancy and Parenting: Outreach Resources to Teens) Home Visitors work with expectant women, mothers of children under age one, pregnant and parenting adolescents, and at-risk youth through personalized visits in the home, school, or community. Staff provide health education and supportive services working in conjunction with other community partners to promote positive outcomes, increased education, and goal achievement.
Essential Duties and Responsibilities
- Provide pregnancy and early childhood education and support through individual visits and referral services.
- Provide weekly visits in an individual or group setting focusing on case management and academic support (with involved teens).
- Partner with school personnel, obstetricians, and other community agencies in the early identification, counseling, and education of pregnant and parenting students.
- Provide information on healthy relationships, decision-making skills, peer pressure, self-esteem and sexuality education through individual sessions.
- Promote public awareness of the RAPPORT Program through speaking engagements at social service organizations, community agencies, healthcare providers, and schools.
Required Education & Experience
- Associate or Bachelor Degree in Human Services, Education, Nursing or other related fields
- Knowledge of prenatal, infant care and education
Travel
Travel within Clearfield County, to and from homes and schools to meet with families who are pregnant or have a newborn.
