Responsibilities and Duties:
- Work collaboratively as part of the ACT Team in order to provide comprehensive care that meets the individual needs of each client enrolled in ACT;
- Assess client health daily and respond appropriately to any noted concerns (i.e. medication side effects, withdrawal symptoms, general health changes, etc.)
- Monitor client use of medications, changes in medications, and assess for efficacy, safety, and/or side effects of medications;
- Collaborate with all providers involved with client’s health care and provide feedback to providers as needed;
- Link client to appropriate healthcare providers or specialists and follow-up with all referrals and recommendations;
- Provide health and wellness education to the client, the client’s supports, and other ACT Team staff members;
- Participate in the formal training and development process with administrative staff and consultants as well as ongoing professional development and continuing education;
- Attend daily team meetings, individual supervision, and group supervision as scheduled;
- Initiate and maintain relationships with community resources, outside human service agencies, and other stakeholders as needed;
- Commit to ensuring fidelity to the ACT model and ongoing quality improvement efforts;
- Complete all required documentation accurately and on time;
- Knowledge, understanding, and experience providing mental health and substance use treatment, including experience conducting mental health evaluations, familiarity with psychiatric medications, and awareness of medication assisted treatment (MAT) practices;
Education and Experience:

- Registered Nurse, licensed to practice without limitations in Pennsylvania
- Experience working in a behavioral health setting preferred
