Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. Responsibilities
- Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient needs.
- Administer medications and injections accurately while adhering to prescribed protocols and safety guidelines.
- Assist with complex procedures such as IV infusion, catheterization, wound suturing, and tube feeding to support patient recovery.
- Maintain detailed and accurate medical records using electronic health record (EHR) systems such as Epic, Cerner, or Athenahealth.
- Provide specialized care for patients with conditions such as Alzheimer's disease, dementia, or those requiring memory care and hospice services.
- Collaborate with interdisciplinary teams to develop and implement individualized discharge planning and case management strategies.
- Ensure infection control practices are followed diligently to prevent the spread of infectious diseases within healthcare settings.
- Support emergency response efforts by managing airway management, ventilator use, and acute pain management in high-pressure environments like Level I trauma centers or ICU units.
Requirements
- Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or jurisdiction.
- Proven experience working in diverse clinical settings including hospital medicine, long-term care facilities, or outpatient clinics.
- Familiarity with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks for medical documentation and patient data management.
- Strong knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation procedures.
- Experience in specialized areas such as critical care (ICU), emergency medicine, geriatrics, or behavioral health is highly desirable.
- Competence in performing phlebotomy, venipuncture, sterile processing techniques, and specimen collection/processing.
- Ability to work effectively with individuals with disabilities or developmental disabilities while demonstrating cultural sensitivity and respectfulness.
