Position Summary:
The Registered Nurse (RN) is responsible for the oversight and delivery of quality Member care and customer service in accordance with established policies and procedures, as well as federal, state, and local regulations. The RN provides direct nursing care, ensures accountability and safety of Members and staff, and promotes Member well-being. Responsibilities include communication with Member families and physicians, supervision of CNAs, and potential weekend supervision of RNs, LPNs, and CNAs.
This position also serves as the Extended Congregate Care (ECC) RN for Members receiving ECC services within the assisted living setting. The RN is responsible for conducting nursing assessments, maintaining accurate Service Plans, and assisting with staff scheduling to ensure appropriate care and staffing levels.
Performance Conditions:
- Must be able to move intermittently throughout the workday, occasional bending or squatting.
- Frequent use of keyboard, repetitive motion, writing, grasping, holding, and reaching.
- Performs light physical work, frequently exerts up to 10 lbs of force and up to 60 lbs on an occasional basis.
- Communicates effectively, both orally and in writing.
- Carries out detailed written or verbal instructions.
- Makes mature judgments, functions independently, is flexible, has personal integrity, and works effectively with GPR personnel, Members, and the public.
- Well-lighted, ventilated work area. Subject to frequent/regular interruptions.
- Requires occasional weekend, evening, or night work, if needed. On-call on an as-needed basis.
Responsibilities:
- Provides direct nursing care to Members under the direction of the Manager and in accordance with established Member care plans, nursing policies, and federal, state, and local regulations. May also receive functional supervision from an RN Supervisor working the shift.
- Delivers nursing services in a timely, respectful, and friendly manner while maintaining Member rights, dignity, and privacy. Maintains a positive and professional relationship with Members, families, visitors, and staff.
- Demonstrates knowledge and application of medication administration techniques, following all standards of practice as set forth by nursing regulatory bodies.
- Leads and coaches Certified Nursing Assistants (CNAs); monitors personnel performance and ensures care standards are met.
- Directs the day-to-day functions of CNAs, including assigning them to specific Members, ensuring compliance with regulations, monitoring quality of care, and ensuring awareness of Member care plans. Provides feedback and makes recommendations regarding CNA activities.
- Maintains appropriate and descriptive documentation in medical records, including but not limited to:
- Accidents/incidents involving Members and physician notification
- Admissions, transfers, and discharges
- Physician’s orders and transcriptions
- Medication sheets and treatment plans
- Reporting discrepancies in care to the Manager
- Ensuring documentation reflects that care plans are followed
- Coordinates the nursing portion of Member Care Plans and Discharge Plans in collaboration with other departments.
- Ensures accuracy of narcotic records and reports any discrepancies immediately according to policy.
- Administers professional nursing services as needed, including infection control procedures, obtaining lab samples (e.g., sputum, urine), recording vital signs, and other clinical functions.
- Participates in the orientation of new Members and their families to the Assisted Living community and continues to engage with them as necessary.
- Assists staff with day-to-day clinical questions and problem-solving as needed.
- Trains and orients new staff members, including but not limited to:
o Job responsibilities and essential functions
o Performance expectations
o Infection prevention
o Safe use of equipment
o Hand hygiene
o Use of PPE
o Universal precautions and bloodborne pathogen protocols
- Assists in lifting or carrying heavy objects, following all safety guidelines and protocols.
- Effectively communicates with Members, families, and staff to ensure coordinated, compassionate care.
- Recognizes and responds appropriately to changes in Member condition, ensuring proper documentation and reporting per department procedures.
- Prepares and administers medications, adhering to the “Five Rights” of medication administration.
- Observes Members for drug effects and interactions and responds accordingly.
- Orders medications as needed, following established pharmacy protocols.
- Actively participates in meetings and required training, including continuing education necessary to maintain licensure and stay informed about changes in regulations and best practices.
- Maintains confidentiality of Members’ Protected Health Information (PHI) and all medical record documentation.
- Performs other related duties as assigned.
Extended Congregate Care (ECC) Specific Duties:
- Serve as the ECC RN responsible for overseeing Members receiving Extended Congregate Care services within the general assisted living community.
- Conduct initial and ongoing nursing assessments for Members requiring ECC services.
- Review and update ECC Service Plans to ensure care is current, compliant, and appropriate to the Member's needs.
- Assist with staff scheduling to ensure proper coverage and staffing levels in alignment with Member needs.
- Serve as the 7:00 a.m. – 3:30 p.m. shift nurse in the assisted living setting, providing nursing care and oversight for all Members, including those receiving ECC services.
Qualifications:
- Must be a graduate from a basic educational program in professional nursing (RN) and have an active license as a Registered Nurse in the State of Florida. Bachelor of Science in Nursing (BSN) preferred. Two (2) years of proven experience in skilled nursing, acute care, or assisted living preferred.
- Proficient in relevant computer software; experience with Electronic Medical Records (EMR) preferred. Strong written documentation skills required.
- Ability to work effectively with geriatric and memory-impaired populations, both individually and in group settings.
- Demonstrated ability to work collaboratively with both professional and non-professional staff.
- Strong supervisory, verbal, written, and interpersonal communication skills.
- Clinical and administrative competence; capable of independent decision-making within policy guidelines.
- Must be empathetic, energetic, and committed to working with a diverse senior population.
