 The Glen Retirement System is seeking an experienced RN to work every other weekend, 2 weekends per month in our long-term care facility, Village Health Care.

ESSENTIAL FUNCTIONS: In physical absence of the Directors of Nursing, responsible for insuring provision of quality resident care in accordance with facility standards and supervising nursing department while on duty. Completion of Administrative Nursing Tasks as Assigned.   Provides direct resident care as assigned or as needed. Will serve as an administrative resource for the Assisted Living nursing department ensuring quality resident care and compliance. 

QUALIFICATIONS/REQUIREMENTS: 
 
  • Registered Nurse – Current Louisiana license
  • Long Term Care experience preferred.
  • Possess communication, time management, and organizational skills.
  • Clinical nursing skills.
  • Management and leadership skills
  • Must be people-oriented individual, have good communication skills, and the ability to work with a diverse group of people.  
  • Must be able to work overtime as required.
  • Computer skills appropriate to job functions
  • Visual & hearing acuity to assess residents’ health status.
  • Finger/hand dexterity to operate small equipment.
 
ADDITIONAL FUNCTIONS:  
 
A.  Administrative
  1. Is responsible for management of nursing department for weekend.  Directing and monitoring to ensure quality resident care in compliance with laws and regulations and in keeping with the standards of The Glen and The Glen’s Mission Statement. 
  2. Will serve as an administrative resource for the Assisted Living nursing department ensuring quality resident care and compliance. 
  3. Has good rapport and communication with Nursing Director, reporting all risk management issues and significant changes with residents’ conditions in a timely manner. 
  4. Is liaison with Director of Nursing, resident’s family members, and physicians. 
  5. Coordinates clinical care with ancillary departments and services. 
  6. Regular rounds on all residents in facility with specific focus on acute medical or behavior problems, condition changes, and risk management issues. 
  7. Communicates resident and family grievances to the Administrator and Director of Nursing. Participates in grievance investigations and resolutions. Review any exception reports and take appropriate action. 
  8. Participates in activities related to facility inspection or investigation by regulatory or accrediting agencies. 
  9. Monitors to assure residents’ rights are protected and residents are free from abuse and neglect. 
  10. Participates as assigned in the facility Quality Assurance and Performance Improvement program. 
  11. May be assigned to assist with information pertaining to the Facility Assessment. 
  12. Assist with the facility’s infection control program as assigned. 
  13. Adheres to a professional code of ethics. 
 
B.  Education 
  1. Participate in the orientation and training of nursing department staff as assigned. 
  2. May in-service staff on outcomes of quality data, infection trending, and interventions for Resident care needs.   
  3. Conduct skills competency evaluations and training as assigned. 
  4. Maintains and improves competencies by participating in and attending professional educational programming. 
 
C.   Clinical Mentor 
  1. Communicates with Household nurses. Offers advice, guidance, and support to address clinical issues and resident or family concerns. Serves as a resource to staff in clinical and managerial decision-making. 
  2. Performs resident assessments and make updates to the resident plan of care as assigned. 
  3. Participates in development of the resident plan of care as assigned. 
  4. Responsible for reviewing incident reports as assigned. Participates in the investigation into root cause. 
  5. Collects data, provides guidance and resources to implement preventive measures and documents feedback. 
  6. Communicates residents’ condition and changes in status to physicians.  Rounds with physician as needed. 
  7. Provides guidance to nursing staff on special care needs including but not limited to IV therapy, wound care, chronic disease care management and monitoring. 
  8. Ensures lab, radiology, and other diagnostic service reports are acted on per facility policy. 
  9. May be assigned to monitor medication rooms, in-room medication storage, medication carts, and emergency pharmacy supply kit for regulatory compliance with state, federal, and pharmaceutical agencies. 
  10. Receives and transcribes physician orders.  
  11. Perform wound care rounds as assigned. Makes assessments and recommendations regarding treatments and preventive measures. Documents measurement, staging, and charts healing progress. 
  12. May be assigned to answer consultant reviews. Communicates trends and takes corrective action. Provides report to the Director of Nursing and Medical Director. 
  13. May be assigned to monitor medication and treatment passes with Household nurses. Provide guidance on regulatory and infection control compliance. Has input on competency. 
  14. Assists with clinical emergencies. 
  15. Handle any first report of injury, seeing that information is complete, correct procedures followed and HR advised. 
  16. Document narrative report for Director of Nursing and place documentation in designated location. 
  17. Tour prospective residents or their families for Village Healthcare. 
  18. Monitor staff performance in all departments. Handle immediate staff concerns and/or prepare report for department head detailing staff issues. 
  19. Assist in the replacement of call-ins for nursing department. Perform charge nurse duties in emergencies, not limited to administering medications and documentation in resident medical records. 
  20. Observe meal service in each dining area in individual households. 
  21. Assist to coordinate weekend activities. Ensure residents attend activities of preference. 
  22. Prepares discontinued medications for donation, performs destruction of discontinued medication, and destroys narcotics with the pharmacy consultant. 
 
Other related projects, duties and responsibilities may be assigned as deemed necessary.
