Since 2004, The Gathering Inn has compassionately reached out to individuals experiencing homelessness, aiming to welcome new beginnings and transform lives. Originally a winter-only shelter serving 40 individuals nightly, we have evolved into a multifaceted organization with multiple locations, serving nearly 500 individuals and families daily. Our mission is to meet people where they are, inspire hope, and walk alongside them on their journey to sustainable housing. Through our emergency shelter, permanent supportive housing, and medical respite care, we provide programs and support designed to empower guests toward self-sufficiency and long-term stability.
Position Overview
The Case Manager is responsible for assuring quality service to all Permanent Supportive Housing residents in a compassionate and professional manner. Overall, the role of a Case Manager is to support and empower our residents to achieve their goals, overcome challenges, and improve their quality of life by facilitating access to the appropriate services and resources. Work should be performed in a cost effective and service-oriented manner as it relates to established organizational standards.
Essential Duties and Responsibilities
- Uphold the Mission, Vision and Values of The Gathering Inn and always maintain a resident-first mentality.
- Conduct comprehensive assessment of residents' needs, strengths, and goals through initial intake. Based on this assessment, develop individualized care plans and goals in collaboration with the residents.
- Maintain accurate and up-to-date records of resident interactions, services provided, outcomes achieved, and any other required documentation in a timely, consistent manner.
- Utilize the Homeless Management Information System (HMIS).
- Coordinate and facilitate access to various service providers and resources to meet identified needs of residents, ensuring access, eligibility, and follow-through.
- Work collaboratively with community partners and resources for seamless delivery of services. This may involve scheduling appointments, arranging transportation, resident advocacy, and making referrals.
- Facilitate bi-weekly house meetings and lead regular life skills classes for residents in the program.
- Understand and maintain resident confidentiality and data privacy standards.
- Keep updated resource information on local and regional human services, with particular attention to services available in El Dorado County.
- Meet with residents weekly to monitor progress, adjust care plans as needed, and provide ongoing support for the duration of their tenancy.
- Work with the PSH Director on non-standard situations to develop an appropriate plan.
- Understand status as mandated reporter of child or vulnerable adult maltreatment.
- Participate in staff meetings, in-service training, and other relevant training.
- Assist residents with daily living needs as required, including but not limited to transportation to medical appointments, grocery shopping, and other errands that support residents' health and stability.
- Assist in mediating minor conflicts or disagreements between residents in a calm, patient, and respectful manner, helping to maintain a harmonious living environment.
- Proactively manage interpersonal dynamics between residents and with neighboring community members to maintain a peaceful and respectful living environment. Address neighbor concerns promptly and professionally to preserve positive community relationships.
- Respond to resident concerns and ensure the safety and wellbeing of all residents.
- Model positive, professional behavior for residents, including appearance and communication.
- Address resident behavior that may negatively impact the household or violate program policies in a calm, supportive, and non-confrontational manner.
Position Expectations
- Be punctual and adhere to staffing schedules.
- Complete work accurately, proofread all documents you are responsible to create or distribute.
- Computer literacy required for documentation and record-keeping purposes.
- Strong organizational skills and a proactive approach to maintaining a neat and orderly work environment. Ability to organize workload effectively, prioritize tasks, and meet deadlines.
- Use good judgment and discretion in all aspects of this position.
- Ability to maintain professional boundaries, confidentiality, and ethical standards.

- Communicate regularly with the PSH Director, Manager and co-workers on all pertinent issues.

- Knowledge of homelessness, poverty, mental health, substance abuse, and trauma.
- Ability to remain calm and composed in high-pressure situations.
- Strong communication and active listening skills.
Travel
- Travel within El Dorado County will be required on a regular basis. Occasional travel to other Counties where TGI programs exist may be required as needed. Therefore, a personal vehicle in good working condition and insured according to Organization policy will be required to fulfill the duties of this position.
- Candidates residing within approximately 30 minutes of Placerville, CA are preferred given the community-based nature of this position and the frequency of local travel required.
Minimum Qualifications
- Associate's degree in Psychology, Social Work, Human Services, or a related field, or equivalent experience in a related area of practice.
- Must have knowledge of social service agencies and experience working in a community-based setting. Knowledge, training, and/or experience working with individuals living with mental illness, substance use disorders, or chronic health conditions is preferred.
- Experience working with older adults (55 ) is preferred.
