A long-term care facility in Middlesex is seeking a dedicated and flexible Physical Therapist (PT) to join our team on a per diem basis, with the opportunity to make a meaningful impact on residents’ mobility, independence, and quality of life.
Currently hiring for Per diem.
Responsibilities:
Evaluate residents and develop individualized treatment plans
Provide therapy to improve mobility, strength, and functional abilities
Collaborate with interdisciplinary team members to support optimal outcomes
Document patient progress in accordance with facility standards
Educate residents and families on treatment plans and exercises
Ensure compliance with safety and regulatory requirements

Qualifications:
Active Physical Therapist license in the State of New Jersey
Prior long-term care experience required
