The Gardens of Easton is seeking a dedicated Social Services Director to lead the Social Services Department within our skilled nursing facility in Easton, PA. This role serves as a liaison and advocate for residents’ social interests and is responsible for planning, organizing, implementing, evaluating, and directing the overall operations of the Social Services Department.
Position Overview
The Social Services Director ensures that residents’ medically related social and emotional needs are identified and addressed through comprehensive assessments, individualized care planning, and ongoing support. This position plays a vital role in resident adjustment, family communication, crisis intervention, and regulatory compliance.
Key Responsibilities
Develop comprehensive social assessments and individualized care plans with measurable goals and interventions
Participate in pre-admission and admission processes
Serve as a resource to residents and families during adjustment periods, crises, or special circumstances
Document residents’ behavioral and psychosocial status accurately and timely
Provide individual and group support services to residents and families
Establish and coordinate a volunteer program to support social service needs
Orient residents and families to the facility and assist with transition and adjustment
Ensure compliance with federal, state, and local regulations governing social services in long-term care
Qualifications
Bachelor’s Degree in Social Work or a related field required
Previous long-term care (LTC) experience preferred
Prior supervisory experience preferred
Strong knowledge of social service regulations and best practices in healthcare
Excellent communication, organizational, and interpersonal skills
Ability to manage multiple priorities in a fast-paced environment
