What are the responsibilities and job description for the Population Health Manager position at The Family Practice & Counseling Services Network?
POSITION SUMMARY:
This position works under the supervision of the Director of Primary Care and in collaboration with the Clinical Directors, Quality Manager, and Revenue Cycle Team. This position leads and manages all activities related to population health management, including care coordination, care gap closure, pay for performance initiatives, and value-based care, etc. The Population Health Manager will identify and track all outcomes and identify areas of improvement and develop best practices related to population health management.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Work with leadership to determine priorities for population health programs, incorporating organizational objectives, revenue opportunities, payer panels, grant obligations, disparities, staffing and other resources and quality metric performance
- Maintain an in-depth level of understanding of how data are captured within the EHR, population health software, reports and other databases in order to assist with end users for reporting and analysis of data
- Manage incoming HEDIS Chart Requests via secure email and/or in person extractor scheduling
- Understands, remains updated and communicates changes/updates related to performance improvement programs, pay for performance and value-based incentives programs
- Meet with payer representatives regularly to understand priorities, payer understanding of FPCSN performance and to develop opportunities for closer collaboration
- Understand HEDIS measures, national recognition programs FPCSN participates in and objectives and commitments of grant-funded programs
- Within priority areas, establish goals, workflows and methods to evaluate progress and outcomes on a regular basis
- Work with Health Informatics Manager and Revenue Cycle Team to assess coding and documentation related to quality improvement and revenue opportunities, maintain coding integrity and provide feedback to clinical directors as needed
- Responsible for Pay for Performance (P4P) incentives for all clinical disciplines (understanding, tracking, communication, reporting)
- Lead and manage the FPCSN Care Coordination program and team
- Perform regular gap analysis for all payor’s CQMs to prioritize workflow improvement interventions
- Work with Accountable Care Organization (ACO) on care coordination efforts
- Assist with grant funded projects and initiatives throughout the network including project management, data reporting, and workflow creation
- Manage Azara DRVS implementation of Abnormal Cancer Screening & Referral modules
- Collaborate with Prenatal team to track and enhance the submission rate of ONAF forms
- Assist the Director of Primary Care with VFC program management and compliance throughout the network
- Contribute to CQI Committee by reporting quarterly progress on VBC & Transitions of Care quality measures
- Train staff as needed
EXPERIENCE AND REQUIREMENTS:
Candidates should possess a diverse range of skills and experience including but not limited to:
Required Education and Experience
- Understanding of healthcare environment through 3-5 years professional experience in population health in a health or human services setting; 3 years in outpatient setting, preferred
- Master’s degree in public/population health or clinical degree or equivalent experience preferred
- Understanding of key population health processes and concepts, including attribution, care gaps, registries, outreach, patient engagement, care coordination, HEDIS metrics, coding
Required Skills and Abilities
- Project management and process improvement experience
- Adept with electronic systems used in healthcare
- Data analysis and/or statistical background skills required
- Strong analytical capabilities and problem-solving skills to analyze and validate data
- Supervisory experience required
- Excellent presentation and communication skills
- Able to work independently
Other Qualifications
- Ability to pass required background screenings.
- Required Immunizations: TB/PPD Test.
- Optional Immunizations: Hep B Vaccination and Flu Vaccination.
- Current valid CPR Certification is required.
- Maintain current valid applicable position credentials.
- Maintain reliable transportation to travel throughout the network and/or participate in events outside the network as needed.
- Must be willing to work on-site.
