Position Summary – RN/LPN MDS Nurse
The RN/LPN MDS Nurse plays a critical role in the coordination and management of the RAI/MDS process. This position provides ongoing clinical guidance to the nursing team, ensures timely and accurate completion of assessments, and supports interdisciplinary care planning in accordance with federal and state regulations.
Key Responsibilities – RN/LPN MDS Nurse
Coordinate and manage the RAI process, including MDS, CAAs, and Interdisciplinary Care Plans
Ensure timely and accurate completion and submission of MDS assessments
Assist with inputting MDS assessments, transmitting to the state, and verifying validation reports
Monitor assessment schedules to ensure regulatory compliance
Schedule and facilitate care plan conferences
Facilitate monthly Quality Measure (QM) meetings
Provide ongoing education and guidance to nursing staff regarding documentation and assessments
Assist in planning, organizing, implementing, and directing nursing department functions
Participate in the recruitment, training, and supervision of nursing staff
Assist with in‑service education to maintain a knowledgeable and compliant nursing team
Qualifications – RN/LPN MDS Nurse
Current RN or LPN license (RN preferred)
Prior experience as an MDS Nurse in a long‑term care or skilled nursing facility
Strong working knowledge of MDS, RAI process, and regulatory requirements
3 years of nurse management or leadership experience preferred
Excellent communication and organizational skills
Demonstrated leadership and supervisory abilities
