We are seeking a motivated and energetic Hospital Liaison to serve as the primary link between hospitals, case managers, discharge planners, physicians, and our skilled nursing facility.

In this role, you will develop and maintain strong referral relationships, educate hospital partners on our services, and help facilitate seamless transitions of care for patients requiring post-acute rehabilitation and skilled nursing services.

• Build and maintain relationships with hospital case managers, social workers, discharge planners, physicians, and healthcare professionals.

• Conduct daily hospital visits and meet with referral sources to promote facility services and specialty programs.

• Evaluate referral opportunities and coordinate with admissions and clinical teams to facilitate admissions.

• Communicate facility capabilities, clinical programs, and outcomes to referral sources.

• Track referral activity, admissions, and marketing efforts.

• Attend networking events, healthcare meetings, and community outreach activities.

• Serve as a resource for patients and families regarding post-acute care options.

• Work collaboratively with facility leadership to achieve census and admissions goals.

• This is a field-based healthcare outreach role involving travel to hospitals and referral sources throughout the county and surrounding areas.

• Healthcare marketing, admissions, business development, or clinical experience preferred.

• Experience in skilled nursing, rehabilitation, long-term care, home health, hospice, or hospital discharge planning strongly preferred.

• Excellent communication, relationship-building, and presentation skills.

• Self-motivated with the ability to work independently and manage a territory.

• Valid driver's license and reliable transportation.

• Bachelor's degree preferred but not required.

We are seeking an experienced Hospital Liaison, or Healthcare Marketing Representative to develop referral relationships with hospitals and healthcare providers. The Hospital Liaison will work closely with case managers, discharge planners, physicians, and admissions teams to increase referrals, support census growth, and ensure smooth transitions of care into our skilled nursing and rehabilitation facility.
