JOB OVERVIEW
The Empowerment Center is seeking a highly organized, dependable, and people-centered Intake Coordinator to manage and coordinate the intake and referral process for individuals seeking behavioral health and supportive services.
This person will serve as one of the first points of contact for individuals, families, referral sources, hospitals, community agencies, and other providers seeking services through The Empowerment Center.
We need someone who can take ownership of a referral and keep it moving.
The Intake Coordinator will receive referrals, communicate with referral sources, gather required records and documentation, schedule intake appointments, organize intake packets, track pending referrals, follow up on missing information, and coordinate with the appropriate clinical and program staff to support timely admissions.
KEY RESPONSIBILITIES
- Receive and process new referrals
- Respond to referral calls, emails, and inquiries
- Complete initial intake and demographic information
- Gather insurance and eligibility information
- Request medical, psychiatric, hospital, treatment, and other supporting records when needed
- Obtain and organize required intake forms, releases, consents, and supporting documentation
- Maintain organized electronic intake and referral files
- Schedule assessments and intake appointments
- Coordinate with LMHPs and other qualified clinical staff when clinical assessments or reviews are required
- Track referrals from initial contact through admission, redirection, or closure
- Follow up consistently on missing documents and outstanding requirements
- Communicate professionally with referral sources and community providers
- Help individuals understand what documentation or appointments are needed to move forward
- Coordinate internally with program staff regarding potential admissions
- Maintain referral and admission tracking logs
- Assist with transition into services once an individual is approved for admission
- Protect confidential information and follow HIPAA and organizational requirements

The Intake Coordinator doesn't have to perform every part of the process personally—but they should know where the referral is, what's needed next, who is responsible, and whether it has been completed.

Pay: $18.00 - $24.00 per hour
