Our client is a lifestyle medicine clinic based in Amherst, NH, specializing in Ehlers-Danlos Syndromes (EDS), hypermobility disorders, and complex chronic conditions such as POTS and MCAS. Rather than fragmented specialist referrals, the practice builds a coordinated, whole-person path forward for every patient — combining diagnostics, movement coaching, patient advocacy, and immersive care intensives. The practice operates on a direct-pay, out-of-network model so clinicians can prioritize personalized, relationship-based care over insurance-driven visit limits.
POSITION SUMMARY
The Ember Collective LLC is running this search on behalf of a confidential client — a lifestyle medicine practice in Amherst, NH (the client's identity is shared with candidates as the process advances). The Nurse Practitioner will provide direct patient care within a multidisciplinary, whole-person care model for patients with EDS, hypermobility spectrum disorders, POTS, MCAS, and related complex chronic conditions. This role is well suited to a clinician who wants to practice relationship-based, complex-case medicine rather than high-volume, insurance-driven visits.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Conduct comprehensive patient assessments, diagnostic workups, and ongoing management for patients with EDS, hypermobility disorders, POTS, MCAS, and overlapping chronic conditions.
- Develop and implement individualized, whole-person treatment plans in coordination with movement coaches, patient advocates, and other members of the care team.
- Prescribe medications and therapeutic interventions within the scope of NH APRN practice authority.
- Provide both in-person and virtual/telehealth consultations.
- Participate in immersive, multi-day care intensives as scheduled.
- Maintain accurate, timely clinical documentation in compliance with applicable recordkeeping and privacy requirements.
- Stay current on emerging research and best practices in connective tissue disorders and complex chronic illness management.
- Communicate clearly and compassionately with patients navigating long diagnostic journeys and multi-system symptoms.
REQUIRED QUALIFICATIONS
- Master's or Doctoral degree in Nursing from an accredited program (FNP, AGNP, or comparable APRN track).
- Active, unrestricted APRN/NP licensure in New Hampshire (or eligibility for licensure), consistent with the NH Nurse Practice Act, RSA 326-B.
- National certification through a recognized certifying body (e.g., ANCC or AANP).
- Current DEA registration, or eligibility to obtain one, appropriate to prescriptive scope.
- Active, unrestricted professional liability (malpractice) insurance, or eligibility to obtain it.
- Strong clinical judgment and comfort managing diagnostically complex, multi-system presentations.
PREFERRED QUALIFICATIONS
- Prior experience with EDS, hypermobility spectrum disorders, POTS, MCAS, dysautonomia, or other complex chronic illness populations.
- Experience or training in lifestyle medicine, integrative medicine, or functional medicine approaches.
- Comfort with direct-pay/concierge-style practice models.
- Experience with telehealth platforms.
Pay: $111,420.09 - $134,183.33 per year
