Our agency’s psychiatric rehabilitation program aims to provide individuals with alternative treatments to prevent hospitalization and legal intervention while maintaining and enhancing their skills to live productively in the community.

POSITION DESCRIPTION:

For an assigned caseload of persons served and under general supervision, provides rehabilitative and case management services with the goal of supporting the person’s served successful community integration. Completes an initial assessment, develops with the person served an individual rehabilitation plan in conjunction with other providers of services and updates this plan according to the established standards. Completes required record keeping and provides a safe environment according to the policy and procedures.

SPECIFIC DUTIES/RESPONSIBILITIES:

  • Responsible for the documentation and maintenance of client’s medical records.
  • Draft Individual Rehabilitation Plans (IRP) two (2) months after initial authorization is completed and then every six (6) months for each person served.
  • Works in collaboration with the program director and the direct service staff to review and update person centered plans (PCP).
  • Perform face-to-face initial Rehabilitation Assessments for all new clients as needed. Must be willing to travel to client’s homes or meet in the community across the Baltimore City and Baltimore County areas.
  • Complete DLA-20 assessments two (2) months after initial authorizations and then every six (6) months for each person served.
  • Works in collaboration with the Psychiatric Rehabilitation Coordinator (PRC) to prepare discharge plans when necessary.
  • Ensures that the program provides rehabilitation activities directed toward development or restoration of skills, including self-care and social skills.
  • Ensures that all services are age-appropriate, including group and self-directed leisure activities, both on-site and off-site.
  • Available for twenty-four (24) hour on-call responsibility and flexibility to be present during crisis situations.
  • Conduct monthly supervision of all PRCs (reviews the PRC’s contact notes and monthly reports).
  • Must be able to report in the office on a part-time basis between Monday – Friday on a schedule agreed upon by the program director.
  • Must work a minimum of 20 hours per week.
  • Participate in all virtual or in-person staff meetings.
  • Reach out to external agencies and the community for new client referrals into the program.
  • Maintain availability for all State and CARF accreditation audits/surveys.
  • All other duties assigned.

ASSESSMENT AND EVALUATION:

  • Review each person’s served IRP to determine goals and objectives.
  • Write progress notes, monthly progress reports, and initial and recurrent IRPs for each person served.
  • Complete DLA 20 Assessment, ensuring that the information coincides with the contact notes, monthly progress reports, initial rehabilitation assessment and IRP, every six (6) months for each person served.
  • Prepare reports for intake, transfer, or discharge meetings.
  • Observe PRC staff and person served interactions.

TRAINING:

  • Collaborate at least one hour per week with therapeutic and rehabilitative staff members to develop and/or improve therapeutic programming or to provide group therapy education classes for participants, if necessary.
  • Participate in-service training sessions as designated by director.
  • Participate in any Crossroads recommended trainings.

QUALIFICATIONS:

  • A Mental Health Professional in the State of Maryland (Board Approved) licensed in one of the following: LMSW, LCSW, LCSW-C, LGADC, LCADC, LGPC, LCPC, Psychologist; Or A Rehabilitation counselor who has a master’s degree in rehabilitation counseling, psychiatric rehabilitation, vocational rehabilitation, or who is currently certified by the Commission on Rehabilitation Counselor Certification or CPRP; Or A therapeutic recreations specialist who has a master’s degree in therapeutic recreation or who is registered as a therapeutic recreation specialist by the National Therapeutic Recreation Society; Or A bachelors prepared Certified Psychiatric Rehabilitation Practitioner currently certified by the International Association of Psychosocial Rehabilitation Services (IASPRS).
  • Must possess a valid Maryland Driver’s License and have a current driving record with less than (3) points on it at the time of hire. Must possess valid automobile insurance coverage. Work will require the use of your own vehicle.
  • Be at least 21 years of age.
  • Obtain certification in cardiopulmonary resuscitation (CPR) and renew the certification annually.
  • Complete first aid training in addition to all other trainings required by COMAR.
  • Be able to communicate with persons served.
  • Be acceptable to persons served.
  • Be able to recognize emergency and be knowledgeable about emergency procedures.
  • Be able to read and write the English language.
  • Must be a US citizen or alien who is lawfully authorized to work in the US.
  • Satisfactory physical examination by a licensed practitioner including, but not limited to freedom from communicable disease as confirmed by an annual PPD skin test/chest x-ray, or documentation from a physician stating that the person is free from communicable disease.
  • Completion of staff required trainings and certifications.
  • Must possess analytical skills sufficient to handle routine administrative details, such as completing incident reporting, case notes, log notes, medication sheets, monthly progress notes, IRPs, billing sheets and any other related documentation.
  • Ability to utilize a computer, specifically including basic word processing skills, internet skills and email use.
