Located in Philadelphia, the Consortium is the premier provider of Supports Coordination, Outpatient Mental Health, Addictions, Case Management, and Functional Family Therapy (FFT) for all ages in West and Southwest Philadelphia, serving approximately 4,000 individuals in 2022. Utilizing Evidence Based Practices (EBP) in all programs, the Consortium is accredited by CARF for outpatient drug and alcohol services and has received the EBP Designation through Community Behavioral Health’s Evidence-based Practice and Innovation Center (EPIC) for both Cognitive Behavioral Therapy (CBT) and Prolonged Exposure (PE) for Post-Traumatic Stress Disorder (PTSD). Most recently, the Consortium’s Integrated Care Program was recognized as “Most Innovative Program” by the City of Philadelphia.

Support Coordinator

Responsibilities:

This ROLE is expected to travel within the region 75% of work hours.

  • Assists the individual in locating and obtaining requisite services/supports, advocate on the individual’s behalf, as necessary. Strives to use natural supports by enhancing individual/family strengths.
  • Contacts individual and family within two (2) workings days of initial case assignment.
  • Identifies the individual’s needs using a person-centered process.
  • Check HCSIS daily for alerts, incidents, updates and changes to caseloads.
  • Maintains accurate service notes in HCSIS.
  • Coordinates annual team meetings according to ISP schedule; prepares invitation prior to scheduled meeting. Ensures that all paperwork in ISP packet, including Prioritization of Urgency of Need for Services (PUNS).
  • Develops an Individual Support Plan (ISP) ensuring that all relevant parties are appropriately involved in the development and implementation of the plan.
  • Develops fiscal year renewal plan.
  • Schedules team meetings.
  • Revises the ISP and PUNS to reflect changes in the individual’s needs and life.
  • Keeps track of services used by individuals to ensure service utilization stays within allocated units of service authorized.
  • Develops budgets for needed services included in the ISP-includes assisting individuals in identifying support providers and respective costs.
  • Assists individuals with obtaining resources, including monitoring proper use of resources.
  • Conducts or coordinates needed assessments—formal and informal.
  • Completes Service Delivery Preference packets and Service Delivery Priority packets for all individuals not receiving waiver funds. Completes waiver packet when funds become available.
  • Completes Re-Determination packet for all individuals receiving waiver funds as directed.
  • Ensures that there is a current annual physical on file for all individuals receiving waiver funds.
  • Participates in an annual assessment and monitoring of Family Driven Support Services (FDSS) for individuals not receiving waiver services.
  • Promotes health and safety of individuals—includes documenting and review of Incident Reports.
  • Completes internal reports for incidents that occur in non-provider settings within 24 hours of notification.
  • Provides educational material and other information to individuals, families, and teams.
  • Identifies gaps in service delivery and reports service delivery needs to supervisor.
  • Maintains a minimum of 75% productivity.
  • Prepares and maintains an up-to-date monthly schedule of appointments in MS Outlook.
  • Participates in the IM4Q process by completing pre-surveys, responding to considerations and follow-up within designated time frames.

What is Expected of YOU

  • Bachelor’s degree OR a High School diploma with 4 years experience
  • Must have at least 12 credits in the social sciences on your college transcript(s)
  • At least 4 years related experience and/or training; or equivalent combination of education and experience required.
  • Valid driver's license, clear driving record required.
  • Proof of insurance and vehicle registration required.
  • Must be able to travel regionally 75% of the time, percentage of travel depends on assigned caseload.
  • Familiarity with working with individuals with developmental disabilities

Job Type: Full-time

Pay: $45,000.00 - $50,000.00 per hour

Expected hours: 37.5 per week

Salary : $45,000 - $50,000
