Role Description
We are seeking a dedicated Home Health Care Manager for a contract-based, hybrid role located in Bronx, NY, with flexibility for remote work. The Home Health Care Manager will oversee daily operations of the home health care services, ensuring high-quality client care and coordination between healthcare providers and clients. Responsibilities include managing client intake, supervising staff, monitoring compliance with healthcare regulations, and facilitating communication between stakeholders. Additional responsibilities include supporting initiatives that align with the organization’s mission to serve communities of color and marginalized populations.
Qualifications
- Strong organizational and administrative skills to manage scheduling, documentation, and overall program operations.
- Experience in care coordination, client assessments, and resource management for home healthcare services.
- Familiarity with healthcare regulations, policies, and compliance standards.
- Excellent communication and interpersonal skills to manage client relationships, support staff, and collaborate with healthcare providers.
- Technical proficiency, including familiarity with healthcare management software and other relevant tools.
- Ability to lead a team and demonstrate problem-solving skills in a hybrid working environment.
- Background in social work, public health, or related healthcare fields is a plus.
- Master’s degree in social work is required.
