Job Summary
The Case Manager plays a crucial role in coordinating resident care and ensuring that individuals receive the appropriate services and support throughout their stay. This position involves working closely with residents, families, and healthcare providers to develop comprehensive care plans, and manage resources effectively. The Case Manager will utilize their expertise in a non-healthcare setting to provide high-quality case management services.
Duties
- Conduct thorough assessments or residents' needs to develop individualized care plans.
- Collaborate with interdisciplinary teams to ensure seamless transitions of care.
- Monitor resident progress and adjust care plans as necessary to meet evolving needs.
- Provide support to residents and families regarding proper care and available resources.
- Maintain accurate documentation of case management activities in compliance with New York State DOH standards.
- Advocate for residents' rights and access to necessary services across various settings.
Skills
- Strong background in case management.
- Experience working with the elderly/aging population.
- Understanding of medical terminology relevant to case management.
