Job Summary
Clark Manor House Enhanced Assisted Living is seeking a per diem LPN, filling in primarily evening shifts with occasional need for days and nights. We are a small house that acts like a big family! LPNs administer all medications and provide all ADLs for our 16 residents.
Must be an LPN with valid NYS License
This is PER DIEM. As needed to fill in, pick up shifts, cover vacations flexibility to work 7-3, 3-11 or 11-7 a plus. May be called to fill in same day when staff fall ill. Potential for permanent hours in the future.
Responsibilities
Recognize and respond to the needs of the residents
Provide and document licensed nursing interventions and treatments as ordered by the physician, in accordance with standard nursing practice, DOH regulatory requirements and facility policy.
Notify RN, Case Manager or on-call RN to report incidents/ accidents or significant changes in the Elder’s clinical condition.
Contact physicians as necessary to report changes in health status of the residents, schedule appointments, clarify information and/or obtain medication/treatment orders, communicate routinely with pharmacist.
Provide direct care to residents as necessary to include personal care, medication administration, assistance with transfers, ambulation, etc.
Monitor and oversee individual medication regimes, report side effects and adverse reactions to RN Case Manager and physician.
Support RN Case Manager with ongoing monitoring and supervision of the facilities medication management plan.
Document personal care provided in accordance with nursing standards of care and regulatory requirements.
Respond to Elder and family questions, concerns and complaints.
Conduct investigations related to complaints and grievances, provide documentation and appropriate follow-up with the physician, Assistant Administrator Case Manager, the resident and their family as well as Administration.
Implement new administrative or clinical interventions and/or protocols to help prevent issues from reoccurring.
Ability to work with and understand the geriatric resident.
Ability to maintain cooperative relationships with the staff and co-workers.
Excellent communication whether in person, fax or telephone.
Ability to remain calm, reassuring and effective
Responsible for charting and reporting any events or changes seen in residents.
Responsible for giving report at shift change to other staff members.
Responsible to provide resident care and hands on care duties as well.
Responsible for answering telephone, fax and email.
Responsible for signing in meds when delivered from pharmacy/family.
Responsible to attend monthly in-service training programs and staff meetings.
Responsible for participation in educational programs as required by the Clark Manor House
Responsible for maintaining confidentiality of residents and the residence.
Responsible for reorders of medications through residents and assistance.
Communicate/ collaborate with licensed or certified agencies as needed to coordinate individual service plans, physician consults or other referrals.
Communicate/collaborate with ancillary support systems such as DME companies or transportation services to accomplish individual service plans (ISP).
Provide flexibility in work schedule with periodic weekend or after hours responsibilities.
Perform other duties as assigned to support the Administrator and Case Manager.

Pay: $29.68 - $34.25 per hour
