Social Worker

CIS's Lambertville senior property is currently looking to hire a Part - Time Social Worker. The social worker plans and coordinates with the necessary discharge planning and coordination of community resources.

RESPONSIBILITIES:

· Assisted with planning and implementing a comprehensive social services program which provides for counseling and other support services for residents and families;

· Identify medical- related social needs of residents, and provide appropriate services to meet the needs of the individual, as well as collective needs of residents, and maintain records relating to the residents’ social work needs and care;

· Consult with other department heads regarding any resident issues, as well as maintenance of appropriate records

· Develop and implement support groups and learning circles that reflect resident populations and needs of the community

· Conduct monthly and as needed resident and family care conferences

· Coordinate with family members and third-party insurance providers regarding residents that are need of managed care assistance when converting to Medicaid

· Recommend care alternatives which may include other community resources, when appropriate

· Maintain and built rapport with residents and family members

· Coordinate and participate in family and resident activities designed to promote social interaction and intellectual stimulation

·Counsel residents and families in dealing with emotional, mental, environmental or physical activities.

· Develops and maintain a list of community resources

· Participates in monthly Quality Improvement Program and consistently reports and tracks all necessary information/trends in regards to community success

Experience:

· Must have 3-5 years related work with senior population or in a health care setting preferred

· Must have knowledge and experience with local, state, and county entitlement programs

· Excellent interpersonal skills

· Excellent written, verbal, and listening skills

· Ability to work independently, implement plans, and have excellent problem- solving skills

· A professional and helpful demeanor both on the phone and in person

· Ability to manage multiple tasks prioritize workload

· Ability to follow written and verbal instructions and work under minimal supervision

Job Type: Part-time
