Description

Vacancy is in the Department of Health and Human Services. This is professional social services to the community through various programs in the Department. The current opening is in Maternal/Child Health whose primary function is to help assess, refer, and link prenatal and postpartum families to improve the health, safety, and overall well-being of families to improve positive birth outcomes and healthy child development. Interviews clients; collects information; assesses information; determines eligibility and may make referrals to other agencies and community support. Offers education and support to promote positive health outcomes. Maintains records and detailed information of cases; verifies information received from clients, and follow up on client's progress.

If selected, the incumbent will participate in programs to become a Certified Child Passenger Safety Technician (CPST) and a Certified Community Health Worker within 18 months.

This is a CHPEA Union position. The hours of work are 40 per week.

This is a grant-funded position renewable on an annual basis.
The City of Hartford is not obligated to fund this position upon termination of funding.

Knowledge, Skills & Abilities

Considerable Knowledge of:
  • Social and medical institutions, clinics and allied cooperating agencies.
  • Social and economic problems of the Hartford area.
Knowledge of:
  • Social casework theory and methods and the techniques of their application to individuals and groups.
  • General social legislation and regulations.
Ability to:
  • Plan, organize and supervise the work activities of subordinates.
  • Interview clients, assess their needs and recommend appropriate action(s).
  • Prepare detailed casework records for client.
  • Complete a variety of forms and documents and handle arithmetic computations.
  • Provide extensive counseling on a variety of subjects.
  • Communicate effectively, orally and in writing.
  • Establish and maintain effective working relationships with clients and outside agencies.

Qualifications

Open to all applicants who meet the following qualifications:

Bachelor's degree from a four-year college or university, with major course work in social services AND a minimum of five (5) years of experience in the delivery of social services to individuals and families.

Whenever possible, an equivalent combination of training and experience will be considered.

Preferred Qualifications: Bi-lingual (Spanish/English)

If selected, you will be required to pass a physical examination administered by a City physician, a drug and alcohol screening examination, and a background check. If appointed, you will serve 3-12 months of probation. This examination and employment process is subject to all federal, state, and municipal laws, rules and regulations.
