The Community Health Center Fellow will support a critical community health initiative by helping to recruit and engage medical specialists to partner with a local Community Health Center (CHC). This fellow willbe employed by the Office of Opportunity and Transformation, but report directly to CHC leadership andplay a key role in expanding access to care for patients by building sustainable relationships with local healthcare providers. This is a unique opportunity to gain hands-on experience in healthcare outreach, community service, and organizational communications.

This part-time fellowship provides a unique opportunity to work closely with Community Health Center leadership and the FrederickCorps program to strengthen access to care for underserved residents. The fellow will support a key community health initiative by researching local medical specialists, conducting targeted outreach, and building sustainable partnerships that expand clinical services for CHC patients. Fellows will gain hands-on experience in healthcare outreach, provider engagement, and community service operations while contributing directly to improved health access in the Frederick community. This is a part-time position, up to 20 hours per week for a period of 16 weeks.

  • Researches local medical specialists and healthcare providers who may be eligible or willing to partner with the Community Health Center.
  • Develops compelling written communications including introductory letters, emails, and outreach scripts that clearly articulate the benefits of partnering with Community Health Centers.
  • Conducts outreach to identified medical specialists via phone, email, and in-person visits as appropriate.
  • Performs consistent follow-up with prospective partners to maintain engagement and move partnerships forward.
  • Creates and maintains a detailed spreadsheet to track all outreach efforts, communication history, responses, and next steps.
  • Collaborates with CHC staff to align outreach efforts with organizational goals and patient needs.
  • Prepares periodic progress updates summarizing outreach activities and partnership outcomes.
  • Creates and delivers a final Capstone Presentation to City leadership that highlights what you learned, key program insights, and recommendations for next steps.
  • Performs other duties as assigned.

REQUIRED EDUCATION AND/OR EXPERIENCE:

  • Current student or recent graduate in public health, health administration, social sciences, communications, or a related field.
  • Experience with outreach, research, community service, healthcare access initiatives, or nonprofit/public-sector operations.

REQUIRED CERTIFICATES AND/OR LICENSES:

  • Vehicle Operator’s license with satisfactory driving record that meets insurability standards of the City’s insurance carrier is required. The employee must maintain the license for the duration of their employment.

REQUIRED SKILLS:

  • Strong written and verbal communication skills; ability to craft clear, professional, and persuasive outreach materials.
  • Strong organizational and task-management skills, including tracking multiple provider contacts and follow-ups.
  • Ability to work independently and collaboratively in a hybrid environment.
  • Proficiency in Microsoft Office Suite, with emphasis on Excel for tracking outreach activities.
  • Comfort initiating cold outreach via phone and email to healthcare professionals.
  • Demonstrated attention to detail and follow-through in managing outreach pipelines.

While performing the duties of this job, the employee is required to move about in an office type setting, to remain in a stationary position for extended periods of time, to compile and retrieve information from computers, telephones phones, etc., to draft and write, to handle paperwork and position self in the course of work.  The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. 

Talking: The incumbent must have the ability to express or exchange ideas by means of the spoken word; those activities where detailed or important spoken instructions must be conveyed to other workers accurately or quickly. 

Hearing: The incumbent perceives the nature of sounds at normal speaking levels with or without correction, and having the ability to receive detailed information through oral communication, and making fine discriminations in sound.
