The Citadel of Glenview is seeking an MDS/Care Plan Coordinator RN to join their team! The MDS/Care Plan Coordinator RN will be coordinating the development and implementation of all resident plans of care in accordance with State & federal regulations and facility policies, communicates each resident’s plan of care to facility personnel via the written care plan, and provides ongoing assessment and needed care plan changes as warranted to meet each resident's needs. MDS/Care Plan Coordinator RN Duties and Responsibilities:Coordinate the completion of each resident’s MDS in a timely manner and in compliance with all State and Federal requirements, maintaining supporting clinical record documentation.Calculate triggers and develop resident assessment protocols for initiation of care plan.Ensure that each MDS accurately identifies the lowest level of functioning allowable to maximize reimbursement opportunities as allowable by Federal and State law. Ensure that CNAs, nurses and others are trained in and have accurately completed required clinical record documentation in order to correctly complete MDS scoring in a timely manner.Ensures Medicare, Medicaid, VA, and benefits carrier claims relating to MDS pending and therapy pending errors are accurate and if not, corrects.Identifies MDS/Census and Accounts Receivable errors for correction as it pertains to facility billing information.Follows and adheres to company’s policies and procedures implementing HIPAA requirements for the privacy and security of protected health information.Perform other duties as assigned.MDS/Care Plan Coordinator RN Qualifications and Skills:Working knowledge of the MDS assessment cycle and assessment information necessary for billing Medicare, Medicaid, HMO, and VA.Experience using computer systems and software including proficiency in Microsoft Office Word and an email system. Strong oral and written communication skills, organizational, and project management skills.Ability to work with all levels of employees. Ability to read, write, speak and understand English. MDS/Care Plan Coordinator RN Education and Experience:Possesses a current RN license to practice in the State as an RN.Skilled Nursing facility experience as an MDS Nurse preferred.Physical Requirements: Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required.Limited potential for exposure to environmental hazards.
