Position Summary: The Wraparound and Special Services Director will be a dynamic, engaging, and creative person who will lead and support teams across our service areas to develop, implement, and support creative strength-based plans for children's and families. Responsibilities
- Engage youth, caregivers, and county stakeholders who need a different approach to serve youth with complex needs, utilizing the strengths of the youth, caregivers, community, and the entirety of the Children's Home service delivery continuum
- Plan and facilitate meetings; identify needs; develop, implement, and support customized strength-based plans; and coordinate a team of support, utilizing creative and persistent problem solving
- Monitors the progression, efficacy, and on-going needs of the youth and their families. Remain responsive to the evolving needs, providing ongoing guidance and coordination of services, advocacy, and connection to resources
- Engage, collaborate, and coordinate with a variety of individuals, external stakeholders, and internal teams, while leading solution focused plans that are compassionate, creative, and consistent – ensuring others "meet the person, not their file"
- Lead, model, and train others in the planning process.
- Complete notes, documentation, and reports in a timely manner.
Education:
- Bachelor's Degree in human services or related field required
- 2 years of experience in foster care, child welfare, or case management required
