Program Overview :
The Exchange Parent Aid (EPA) intervention is an evidence-supported service model forming part of a comprehensive, statewide family support services continuum that will provide supportive and educational in-home services for parents/caregivers. EPA will provide parents/caregivers with individualized family support, effective parenting skills and strategies, and teach them about enhanced social connections, social-emotional competency, child safety, and child development.
Position Overview:
The position of Care Coordinator serves an important role in our agency’s mission to prevent child abuse. As a member of our team, the Care Coordinator will work alongside the Parent Aide to connect families to needed resources in the community and provide individual support to ensure successful service linkage. The Care Coordinator will act as a resource broker, by building a relationship with the community, collecting community-based resources, and linking families to supports that will enhance their protective factors
Work Location: Home Based (with travel to the office and throughout the assigned New Jersey communities).
Duties Include:
- Collaborate with Parent Aide and Program Supervisor to identify immediate service needs for families.
- Develop and maintain relationships with a diverse array of community programs.
- Collaborate with families, Division of Child Protection and Permanency (DCP&P), and community stakeholders to identify viable resource options.
- Participate in advisory councils/boards within their local community/area of service.
- Complete required documentation, forms, and produces statistical reports.
- Submit monthly/quarterly progress reports focusing on process and outcome indicators.
- Participate in agency activities, required meetings, and trainings as mandated by the agency and Department of Children & Families.
- Regularly research and document community resources including services related to health, housing, education, employment, and social supports.
Education and Experience:
Bachelor’s degree in Psychology, Social Work or equivalent experience within the child welfare system preferred. Must have a valid driver’s license and reliable transportation. capabilities preferred
