Job Overview
As a Bilingual Care Manager, you will play a vital role in supporting individuals and families facing diverse challenges by ensuring they receive the resources, advocacy, and guidance needed to thrive. You will provide culturally competent care in both English and Spanish, empowering clients through personalized support and collaboration. Your work will involve care coordination, crisis intervention, and motivational coaching to help clients achieve stability, wellness, and independence.
Responsibilities
- Conduct comprehensive assessments to identify client strengths, needs, and goals, developing individualized care plans.
- Provide bilingual support (Spanish and English) to clients and families, ensuring clear communication and culturally sensitive service delivery.
- Offer crisis intervention and case management services to ensure timely assistance and stabilization.
- Coordinate with healthcare providers, community organizations, and social service agencies to connect clients with appropriate resources (e.g., housing, childcare, workforce development, benefits assistance).
- Advocate for clients within medical, behavioral health, and social service systems to ensure equitable access to care.
- Monitor client progress, update care plans, and maintain accurate, timely documentation in compliance with agency and regulatory standards.
- Facilitate engagement and empowerment through motivational interviewing and strength-based approaches.
- Participate in interdisciplinary meetings and team collaborations to promote continuity and quality of care.
Qualifications
Required:
- Bachelor’s degree (BA or higher)
- 2 years experience working with children
- Fluency in both Spanish and English (spoken and written).
- Experience working with diverse populations, including families, children, and individuals with behavioral health or social support needs.
Preferred:
- Experience in care coordination, case management, or care management.
- Familiarity with community resources, including Medicaid/Medicare, Section 8 housing, and child welfare systems.
- Knowledge of New York State or similar care management programs.
Job Type: Part-time
Pay: $250.00 - $500.00 per month
Expected hours: 1 – 2 per week
