Summary

The Child Center of NY is seeking a Licensed Mental Health Professional to join our Assertive Community Treatment (ACT) team for youth and families. The Youth ACT program provides intensive, community-based services to children and adolescents with serious emotional disturbance and their families.

The Licensed Mental Health Professional is responsible for delivering individualized, evidence-based treatment to both the child and their caregivers, addressing the complex clinical and systemic needs of the family unit. This role involves providing direct therapeutic services, collaborating with an interdisciplinary team, and engaging families in the home and community to support sustained recovery and improved functioning.

Essential Functions

  • Provide direct therapeutic services to clients and families using a variety of clinical modalities (individual, family, and group interventions).
  • Conduct comprehensive assessments and develop treatment plans tailored to each child and family’s needs.
  • Implement evidence-based practices to address identified clinical, behavioral, and family system challenges.
  • Facilitate linkage to community resources and natural supports to strengthen the family’s ability to sustain progress.
  • Perform home and community visits as the primary setting for care delivery.
  • Maintain accurate and timely documentation in the agency’s Electronic Health Record (EHR) in compliance with city, state, and federal standards.
  • Participate in daily team meetings and weekly supervision to review cases, monitor progress, and ensure quality of care.
  • Provide consultation and collaboration with schools, ACS Child Protective Specialists, hospitals, and other service providers as appropriate.
  • Participate in case reviews and multidisciplinary treatment planning meetings.
  • Share in rotating 24/7 on-call coverage to provide crisis response and support to families.
  • Meet program quality standards and data metric requirements related to service delivery, documentation, and outcomes.
  • Perform other related duties as assigned.


Minimum Qualifications

  • Master’s degree in Social Work, Mental Health Counseling, Creative Arts Therapy, or related field from an accredited institution.
  • Current New York State licensure (LMSW, LMHC, LCAT).
  • Experience working with children, adolescents, and families with serious emotional disturbance or behavioral health needs.
  • Strong engagement, crisis intervention, and family therapy skills.
  • Proficiency in Microsoft Office and Electronic Health Records.
  • Ability to work independently while contributing to a collaborative team model.
  • Flexibility to provide services in the community, including homes, schools, and other settings.
