Master Social Worker - Fee for Service

The Fee for Service Social Worker is responsible for providing individual and group counseling and direct service to children, teens, adults and their families. Services include screening and assessment, treatment planning, provision of counseling/psychotherapy and crisis intervention. The Fee for Service Social Worker is a member of the treatment team and collaborates with other team members.

Essential Functions:

• Provide direct patient care to clients using a variety of social work and therapeutic modalities in order to help them maximize their level of functioning.
• Maintain appropriate professional patient care documentation in the agency selected Electronic Health Record (EHR), including client assessment and individual notes in compliance with all city, state and federal standards of practice.
• Facilitate the client’s utilization of available support systems and community resources to meet needs identified in clinical evaluation or treatment planning.
• May perform home visits as deemed appropriate or necessary by the treatment plan.
• May conduct consultations with ACS Child Protective Specialists or other internal or external staff on concerns they may have with clients.
• Meets data metric requirements, quality of care expectations using traditional social work interventions, or evidence-based models.
• Performs other related duties as assigned.

Minimum Qualifications:

• Master’s Degree in Social Work from an accredited School of Social Work.
• Bilingual in one of the following languages: Bengali, Cantonese, Madarin, Spanish - others may be required depending on the needs of the clients
• Availability for Position: Willing to work flexible schedule and possibly on Saturdays.
• Proficiency in Microsoft Office.

Qualifications Preferred:

• LMSW or LCSW certification.
• Proficiency in Behavioral Health Electronic Health Record
