These services range from assessment, treatment planning, and provision of counseling/psychotherapy and crisis intervention. Summary

Essential Functions:
- Provide direct patient care to clients using a variety of social work modalities in order to help them maximize their level of functioning.
- Maintain appropriate professional patient care documentation in the agency selected Electronic Health Record (EHR), including client assessment and individual notes in compliance with all city, state and federal standards of practice.
- Facilitates the client’s utilization of available support systems and community resources to meet needs identified in clinical evaluation or treatment planning.
- Meet on a weekly basis with supervisor to discuss achievement of program targets, client goals, quality of client care and reporting requirements.
- May conduct consultations with ACS Child Protective Specialists or other internal or external staff on concerns they may have with clients.
- May attend case reviews and develop next steps for the families and clinicians
- Meets data metric requirements, quality of care expectations using traditional social work interventions, or evidence-based models.
- Performs other related duties as assigned.
- Master’s Degree in Social Work or Marriage and Family Therapy from an accredited institution.
- Bilingual in one or more of the following: Spanish, Creole, Bengali, Mandarin, Cantonese, and others, may be required depending on client needs.
- Proficiency in Microsoft Office.
- Ability to plan and carry out assignments independently.
- Ability to prioritize, adhere to timelines and multi-task.
- LCSW or LMFT
- Proficiency in Welligent.
- Creating and maintaining a safe space with children and their family members.
- Maintain regular consistent and professional attendance, punctuality, personal appearance, and adherence to relevant health & safety procedures.
- Safe and successful performance, including meeting productivity standards.
- Maintain an understanding of the principles, methods and processes needed to perform the job. Attends staff meetings, seminars and in-service training as assigned.
- Demonstrates the ability to complete work in an acceptable manner by the time and/or date established with accuracy, neatness and thoroughness and minimal amount of errors.
- Maintain established departmental policies and procedures including maintain strict confidentiality and adherence to all HIPAA guidelines at all times.
