The Case Manager provides comprehensive, client-centered case management services to individuals experiencing homelessness who are living with HIV or are at increased risk of HIV infection. The position supports clients in accessing healthcare, prevention services, stable housing, and community resources while promoting health, self-sufficiency, and overall well-being. Services are delivered using a trauma-informed, strengths-based approach that respects client dignity and self-determination.


Key Responsibilities:

  • Client Assessment: Conduct thorough assessments to identify clients’ needs, strengths, and barriers.
  • Service Planning: Create and implement Individual Service Plans (ISPs) with measurable goals, timelines, and strategies tailored to each client.
  • Resource Coordination: Link clients with essential services including housing, healthcare, substance use treatment, mental health services, and income supports.
  • Advocacy & Empowerment: Advocate for client access to services while fostering their ability to advocate for themselves.
  • Monitoring & Evaluation: Monitor client progress through regular follow-up, modify plans as needed, and ensure appropriate levels of care and support.
  • Documentation & Compliance: Maintain accurate, timely, and confidential case notes and records in accordance with agency policy and funding requirements.
  • Collaboration: Coordinate with team members, healthcare providers, social service agencies, and community partners for integrated service delivery.
  • Client Education & Outreach: Educate clients about HIV care, treatment adherence, and available community resources. Participate in outreach activities and events as needed.
  • Crisis Response: Provide timely and appropriate crisis intervention and de-escalation as necessary.


Qualifications:

  • Bachelor’s degree in Social Work, Human Services, Public Health, or related field preferred. Equivalent relevant experience considered.
  • Minimum 2 years’ experience in case management or social services, ideally working with individuals experiencing homelessness, or living with HIV/AIDS.
  • Knowledge of local service systems, HIV-related care, and trauma-informed practices.
  • Excellent verbal and written communication skills.
  • Strong organizational skills and ability to manage a full-time caseload independently.
  • Commitment to client confidentiality, dignity, and empowerment.
  • Bilingual (English/Spanish) strongly preferred.

  • Work Environment & Schedule:

    • Full-time, 35 hours per week.
    • Flexible schedule may include occasional evenings or weekends to meet client needs.
    • Travel within the community may be required (mileage reimbursement provided where applicable).


    Pay: $54,600.00 per year
