What are the responsibilities and job description for the LPN Case Manager position at The Center for Violence Prevention?
The LPN Case manager is responsible for providing trauma-informed, client-centered medical support to individuals residing in the Center for Violence Prevention’s shelters who have experienced domestic violence, sexual assault, or human trafficking. The LPN will complete medical intakes, monitor basic health needs, support medication management under direction of a NP or RN, and provide education on health and wellness. The LPN works closely with shelter staff to ensure that clients receive appropriate medical referrals and support while maintaining a safe and healing environment.
Responsibilities:
· Conduct medical intakes for new clients, including obtaining health history, current medications, allergies, and baseline assessments within LPN scope of practice.
· Provide basic first aid and non-emergency medical care within scope of practice.
· Assist clients in managing prescribed medications, including documentation and safe storage, according to established policies.
· Facilitate communication and calls to the Registered Nurse (RN) to ensure timely response to sexual assault cases and coordination of appropriate medical care.
· Support survivors during medical appointments by providing advocacy, emotional support, and health education.
· Maintain accurate, confidential medical records in compliance with HIPAA and shelter policies.
· Participate in multidisciplinary team meetings to share health-related updates and contribute to client care plans.
· Assist with inventory and organization of medical supplies.
· Adhere to shelter policies, procedures, and infection control practices.
· Perform other related duties within LPN scope of practice as assigned.
Required Background:
· Current LPN license in good standing.
· Prior experience in community health, women’s health, or trauma-informed care preferred.
· Strong communication and organizational skills.
· Ability to work independently and as part of a team.
· Must pass a criminal background check.
· Valid driver’s license and reliable transportation preferred.
Required Skills:
· Ability to read, write, and document clearly in medical records.
· Strong assessment skills within LPN scope of practice.
· Ability to read, write, and speak English;
· Ability to understand and work within the Team Model;
· Understands the dynamics of trauma-informed care and has the ability to identify and respond to victims of violence;
· Demonstrates the ability to exercise and model healthy boundaries with clients and others;
· Maintains professional working relationships with staff and program stakeholders;
· Possesses strong problem solving and conflict resolution skills;
· Ability to work independently with minimal supervision;
· Ability to operate a computer, multi-line phone, and other office equipment;
· Possess and have full use of reliable transportation with valid automobile insurance.

Pay: $40,000.00 - $45,000.00 per year
