Registered Dietitian
Pay Rate: $30.00-$35.00 per hour
Monday and Friday with Flexible Schedules
Responsibilities
- Assess the Clinical nutrition needs of residents/patients and develop a plan of care within the electronic medical record.
- Develop, Implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends , intake and comorbidities.
- Provide nutrition education and counseling to resident, families and staff.
- Collaborate with interdisciplinary teams to develop a nutritional health plan for the patient's overall wellbeing
- Adhere to facility policy and procedures and to CMS, State and Federal regulations.
- Assist and participate interdisciplinary care conferences.
- Assist dietitian in duties as assigned.
- Qualifications
- Registered by the Commission on Dietitetic and Registration
- Hold a Certification / License in good standing within the state of practice
- Knowledge of the state survey process and compliance with nutrition care regulations
- Strong organization and communication skills
