Responsibilities
Provide direct patient care to residents in our Subacute Rehab wing, including assessment, planning, implementation, and evaluation of care plans
Collaborate with interdisciplinary team members to develop and implement individualized care plans
Administer medications, treatments, and therapies as ordered by physicians
Monitor and record patient vital signs and medical conditions
Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
Maintain accurate and up-to-date patient records
Participate in quality improvement initiatives to enhance patient care and outcomes
