CASES is seeking a committed and compassionate Registered Nurse to join our Forensic Assertive Community Treatment (FACT) team. As part of this mobile, multidisciplinary team, you’ll help deliver a full spectrum of integrated services that support recovery, stability, and independence. FACT clinicians work together to help clients secure housing, build meaningful relationships, manage symptoms, pursue employment, and improve overall quality of life.
In this role, you’ll provide medication management and administration, conduct health assessments, offer counseling and psychoeducation, and promote wellness and recovery. You’ll practice within the core principles of ACT, professional nursing ethics, and regulatory standards; while enjoying the autonomy and variety that community‑based work provides.

Shift Hours: This position is full-time Monday – Friday from 9:00 AM – 5:00 PM.

What You Will Be Doing:
- Deliver at least 80% of nursing services in the community, including medication administration (oral and injectable), health assessments, psychoeducation, and monitoring of medication side effects and adherence.
- Conduct health screenings and coordinate care with primary care, specialty providers, hospitals, shelters, housing providers, and other service systems.
- Collaborate with prescribers, Licensed Practical Nurses (LPNs), and multidisciplinary team members to support participant health and recovery goals.
- Complete timely and accurate documentation, including progress notes, assessments, and incident reports, in compliance with OMH, DOHMH, and Medicaid standards.
- Use trauma-informed, culturally responsive, and recovery-oriented approaches in all services.
- Participate in team meetings, case conferences, and on-call rotations as assigned.
- Promote integrated care and improved health outcomes for participants with complex medical and behavioral health needs.
- Help ensure we maintain a safe working environment by reporting any hazards or unsafe conditions, e.g., liquid spill in the hallway, to the appropriate facilities staff member or supervisor.
- Any other duties as required by Team Leader and senior clinical leadership.
- Current New York State Registered Nurse (RN) license.
- Minimum of 2–3 years of experience working with adults with serious mental illness, co-occurring substance use disorders, trauma, homelessness, and/or criminal justice involvement.
- Knowledge of community treatment models, support services, and resources for justice-involved and high-need populations.
- Proficiency with electronic health records and basic computer skills.
- Strong organizational skills, self-motivation, and ability to work independently in the field.
- Commitment to recovery-oriented, trauma-informed care and social justice principles.
- Individuals with lived experience are encouraged to apply.
- Fluency in Spanish.
