Description: The primary purpose of this position is to be a member of a patient’s care team in order to assist in planning, developing and coordinating patient care in accordance with applicable federal, state and local standards, guidelines, regulations to assure that the highest degree of quality care can be maintained at all times.
Primary Job Duties and Responsibilities:
- Meets with administration, medical and nursing staff, as well as other related departments in planning services-to include patients and family
- Partners with the patient, family members, patient’s care team and social services staff, health and community agencies to insure a smooth transitions upon discharge from the Center
- Ensures that all documentation is informative, descriptive and follows the Center’s policy and procedures
- Assists with the admission process when needed
- Attends care plan meeting to provide insight and professional guidance to patients needs
Education/Certifications:
- Bachelors in Social Work (preferred)
- Must meet the State educational requirements for a skilled nursing facility
Experience and Specific Requirements:
- 1 years’ experience in a skilled nursing facility and/or a long term care facility (preferred)
- Must possess the ability to make independent decisions, follow instructions and accept constructive criticism
