We are seeking an experienced, motivated Clinical Supervisor (RN) to join our team. The Clinical Supervisor (RN) provides clinical oversight for the SNF unit and is responsible for coordinating hospital-type admissions and discharges, ensuring accurate clinical documentation, timely physician communication, and continuity of care.

Responsibilities:

• Coordinate all SNF admissions, ensuring readiness, documentation, and clinical appropriateness.
• Provide a welcoming admission process for residents/families, answering questions and setting expectations.
• Verify admission orders, medication reconciliation, initial assessments, and timely care plan initiation.
• Serve as the primary clinical liaison with hospitals, case managers, residents, and families for smooth transitions.
• Coordinate safe, organized discharges including instructions, education, and follow-up arrangements.
• Ensure timely physician communication and accurate order implementation for admits, discharges, and condition changes.
• Monitor transition-related documentation for accuracy/compliance and address deficiencies.
• Partner with the interdisciplinary team to meet resident needs and support discharge goals.
• Provide clinical guidance to nursing staff related to admissions/discharges and resident safety.
• Promote excellent customer service by promptly addressing concerns and improving the resident/family experience.
