Role Description: This is a full-time, on-site Care Manager role located in Pearl City, HI. The Care Manager will coordinate and oversee services for clients, conduct in-person evaluations, and monitor changing needs to ensure appropriate services and support are in place. Responsibilities include collaborating with families, healthcare providers, and social service agencies; documenting client information; and maintaining clear communication about care goals and progress. The role includes management of individual finances, taxes, and legal issues related to Estate Management, Guardianship, Conservatorship, and Trust Administration. The Care Manager is expected to maintain high ethical standards, respect client autonomy, and contribute to a supportive team culture.
Qualifications
- Case Management skills to assess needs, develop care plans, coordinate services, and track outcomes.
- Experience in Elder Care and Social Services to support older adults and vulnerable individuals.
- Background in insurance, finances, taxes, and government benefit programs is desired.
- Foundational knowledge of legal issues involved with Guardianship, Conservatorship, and Estate Management is desired.
- Strong interpersonal, communication, and conflict-resolution skills for working with clients, families, and multidisciplinary teams.
- Organizational and documentation skills to manage caseloads, maintain accurate records, and comply with regulatory and ethical standards.
- Ability to work independently on-site, exercise sound judgment, and navigate sensitive situations with discretion and empathy.
- Relevant education in social work, gerontology, nursing, human services, or a related field; licensure or certification in a care-related discipline is a plus.
