Key Responsibilities
- Process and coordinate new patient referrals, ensuring accurate and timely intake documentation and follow-up
- Communicate with referral sources, patients, families, and clinical teams to facilitate admissions and ongoing care coordination
- Verify insurance benefits, authorizations, and coverage information while maintaining accurate patient records in the EMR
- Schedule admissions, nursing visits, and other clinical services while monitoring referral and intake workflows to meet service timelines
- Provide exceptional customer service and administrative support by answering phones, managing inquiries, and ensuring seamless communication across departments
Qualifications:
- High school diploma or equivalent required; additional education preferred
- Two or more years of healthcare intake, hospice, home health, or related healthcare experience preferred
- Knowledge of medical terminology and healthcare operations preferred
- Experience with EMR systems and referral platforms such as Homecare Homebase, CarePort, NaviHealth, Allscripts, or similar systems preferred
- Strong communication, organizational, and multitasking skills
