We are hiring an experienced and compassionate Hospice Registered Nurse Case Manager (RNCM) to support patients throughout Berks County and surrounding areas. We are hiring an experienced and compassionate Hospice Registered Nurse Case Manager (RNCM) to support patients throughout Berks County and surrounding areas. This full-time role offers a balanced daytime schedule, strong team support, and the opportunity to build meaningful relationships with patients and families through continuity of care.
Schedule
- Full-time, Monday–Friday
- 8:30 AM – 4:30 PM
- Rotating backup on-call (primary on-call RN handles calls — you support only when needed)
- Earn additional compensation when you take on-call shifts

- Provide compassionate nursing care to hospice patients in homes and facilities
- Case manages a defined patient panel with holistic attention to comfort and dignity
- Build meaningful patient and family relationships during end-of-life care
- Educate patients and caregivers on symptom management and comfort measures
- Coordinate closely with physicians, aides, social workers, chaplains, and leadership
- Participate in interdisciplinary team meetings and care planning
- Ensure accurate, timely EMR documentation and communication
- Maintain strong clinical judgment, compassion, and professionalism
Professional Experience/Qualifications:
- Maintains the agency's mission, philosophy and core values.
- Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management.
- Always maintains patient confidentiality, including all HIPAA regulations.
- Licensed or registered in the state in which currently practices
- Must pass a criminal background check & MVR check.
- Completed health statement acknowledging ability to perform the duties of the position.
- Valid state drivers license. Proof of current automobile insurance.
- Current CPR card;
- TB testing per agency policy;
