Schedule, Hours, & Territory
- Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
- Greater Indianapolis, IN area
- Opportunity to provide meaningful hands-on care
- Strong nursing leadership and team support

- Provide nursing care as delegated by the RN
- Monitor patient status and report changes
- Support comfort and symptom management
- Document care provided in the EMR
- Collaborate with the interdisciplinary team
- Active Licensed Practical Nurse (LPN) license
- Hospice or long-term care experience preferred
- Active driver’s license
- Ability to pass a criminal background check
- Ability to pass a Motor Vehicle Record (MVR) check
- Current TB testing in accordance with agency policy
- Current CPR certification as required for the role
