Schedule
- Monday-Friday
- Daytime hours (generally 8:30 am - 4:30 pm)
- No nights, weekends, or holidays

- Ability to pass a background check
- Hospice or home care experience preferred
- Strong communication skills and compassionate approach
- Ability to work independently and as part of the interdisciplinary team
- Provide personal care and comfort to hospice patients in homes and facilities
- Assist with bathing, dressing, grooming, toileting, transfers, and mobility
- Offer emotional support and companionship
- Report changes in condition to the nursing team
- Document services provided and follow the established plan of care
- Support dignity, comfort, and quality of life at every visit
Qualifications
- Maintains the agency's mission, philosophy, and core values.
- Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management.
- Always maintains patient confidentiality, including all HIPAA regulations.
- Licensed or registered in the state in which currently practices
- Must pass a criminal background check & MVR check.
- Completed health statements acknowledging the ability to perform the duties of the position.
- Valid state drivers license.
- Proof of current automobile insurance.
- TB testing per agency policy
- High School Diploma or GED
- Graduate from an approved training and competency course.
