Hospice Registered Nurse (RN) Weekend Warrior

Join our team as a Weekend Warrior RN, supporting patient care from Friday through Monday. This full-time position offers flexibility in scheduling. For example, a typical schedule may start around 2:30 PM on Friday and continue through Monday morning (around 10:00 AM), but we are open to adjusting hours to fit the right candidate while maintaining weekend coverage.
Schedule, Hours & Territory:
- Weekend coverage from Friday through Monday with flexible start and end times to fit your schedule.
- This role includes on-call responsibilities, along with completing scheduled visits as needed.
- Territory can span from West Branch to Gaylord, MI.

Key Responsibilities:
- Conduct comprehensive patient assessments and develop individualized plans of care.
- Provide skilled nursing care in accordance with hospice standards and physician orders.
- Coordinate care with physicians, caregivers, and interdisciplinary team members.
- Educate patients and families on symptom management, disease progression, and end-of-life care.
- Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB (Homecare Homebase).
- Participate in interdisciplinary team meetings and care planning.
- Support after-hours patient needs as part of an on-call rotation when applicable.
Requirements:
- Active Registered Nurse (RN) license.
- Hospice or home health experience preferred.
- Strong clinical assessment and care coordination skills.
- Ability to work independently in the field.
- Strong communication and documentation skills.
- Experience using HCHB (Homecare Homebase) preferred.
Field Role Requirements:
- Active driver's license.
- Ability to pass a criminal background check.
- Ability to pass a Motor Vehicle Record (MVR) check.
- Current TB testing in accordance with agency policy.
- Current CPR certification as required for the role.
