The Care Team Hospice is seeking a compassionate and experienced PRN Nurse Practitioner to support our growing hospice program. This role offers the opportunity to make a meaningful difference by providing face-to-face assessments, symptom management, and clinical expertise for patients with life-limiting illnesses while working alongside an experienced interdisciplinary team.
As a Hospice Nurse Practitioner, you'll partner with physicians, nurses, social workers, and other clinicians to evaluate patient eligibility, support quality end-of-life care, and ensure patients receive compassionate, patient-centered services. This position is ideal for someone who values autonomy, enjoys building relationships with patients and families, and is passionate about palliative and hospice care.
Compensation
• Paid $130 per Face-to-Face (F2F) Visit

Qualifications:Key Responsibilities
• Complete required Face-to-Face (F2F) encounters for hospice recertification prior to the third benefit period and each subsequent benefit period.
• Perform comprehensive patient assessments, evaluate disease progression, and communicate findings to the Hospice Medical Director and interdisciplinary team.
• Collaborate with physicians, nurses, and other team members to support symptom management and individualized plans of care.
• Educate patients, families, and caregivers regarding disease progression, symptom management, medications, and end-of-life care.
• Accurately document all patient assessments and clinical findings in accordance with regulatory and hospice requirements.
Qualifications
• Current Nurse Practitioner (APRN/NP) license in the state of practice.
• Graduate of an accredited Nurse Practitioner program.
• Minimum of two years of Nurse Practitioner experience; hospice, palliative care, home health, geriatrics, or primary care experience preferred.
• Strong clinical assessment, communication, and critical thinking skills.
• Knowledge of hospice philosophy, palliative care, symptom management, and end-of-life care preferred.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Valid driver's license, reliable transportation, and current automobile insurance required.
