The Care Team Hospice is seeking a compassionate and patient-focused Hospice RN Weekend Warrior to join our team. This is a rewarding opportunity for a nurse who is passionate about providing exceptional end-of-life care while building meaningful relationships with patients and families.
As a Weekend Warrior, you will coordinate patient care, manage individualized plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life. In this role, you’ll have a mix of scheduled patient visits seeing patients in their home or in a facility, support the on-call team as needed, and have the opportunity to expand your skills by learning back-office functions to support branch operations.

Qualifications
- Active RN License required
- Hospice nursing experience preferred
- Experience with Homecare Homebase (HCHB) preferred
- Strong assessment, communication, and care coordination skills
Schedule & Work-Life Balance
- Friday to Monday - we are flexible on start time for Friday afternoon and end time for Monday morning. It will be a mixture of scheduled visits and assisting the on-call team, with the possibility of learning back-end office assistance.
- On-call opportunities available with additional compensation
- Some locations offer dedicated on-call teams, resulting in backup coverage only
Key Responsibilities:
- Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care.
- Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB).
- Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
- Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
- Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
