We're seeking a compassionate Part-Time/PRN Social Worker to provide emotional support, care coordination, and valuable resources to patients navigating their hospice journey. Schedule, Hours & Territory
• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM EST
• Crown Point, IN, and a 1-hour radius surrounding (Mileage reimbursement applicable)

Key Responsibilities
• Conduct psychosocial assessments
• Provide counseling and emotional support to patients and families
• Assist with advanced care planning and coordination of community resources
• Participate in interdisciplinary team meetings
• Document services accurately and timely in the EMR HCHB (Homecare Homebase)
Salary : $28 - $36
